The 200-hall shower room has been unusable since at least November, with water spraying from broken pipes and a medical glove taped over the plumbing to catch drips. Maintenance Director #295 ordered a shower valve on November 12 and a vacuum breaker on December 2, but told inspectors on December 30 he wasn't sure when repairs would be completed.

"He also had other repairs to complete," according to the inspection report.
Certified Nursing Assistant #246 told inspectors that residents had to be moved to other shower rooms because the 200-hall "sprayed water out of the pipes and was not working properly." But those alternatives created new problems.
The hot water handle in the 400-hall shower room sink produced no water at all. When inspectors tested the shower itself, water measured 101 degrees Fahrenheit. The 500-hall shower registered 102 degrees.
Both temperatures are too cold for safe resident bathing.
CNA #246 confirmed "the water temperature in the shower rooms on the 400 and 500 halls were always cold."
The 200-hall shower room showed additional maintenance failures. The ceiling exhaust fan cover was missing entirely. Clear liquid and brown stains filled the ceiling light cover, which the maintenance director acknowledged appeared to be water damage.
When asked about temperature monitoring, Maintenance Director #295 admitted "he did not check water temperatures in the shower rooms."
The violations affected 56 residents living on four units of the 76-bed facility. Federal inspectors responded to a complaint filed against the nursing home.
Water continued dripping into the makeshift medical glove even when the 200-hall shower was turned off completely.
