The incident occurred on September 19, 2025. The resident, identified in the inspection report as R1, had taken and spilled a peer's coffee. The peer swung back at him, and R1 fell. He had swelling to his right arm and discoloration to his right eye. He was placed on pain medication while staff arranged an x-ray.
A nurse supervisor called R1's family that evening to explain the situation, noting the x-ray was pending and pain medication had been given. The x-ray results came back the following morning, September 21. The arm was fractured.

When inspectors interviewed V8, a certified nursing aide, on October 22, she said she worked both the day and evening shifts on September 19 but denied being assigned to the eighth-floor unit where R1 lived. The facility's own staffing assignment sheet from that date listed her on the eighth floor for both shifts. Her initials appeared next to her name on the sheet.
She had signed the document. She denied being there.
A licensed practical nurse who worked a double shift beginning the night of September 20 confirmed to inspectors that R1 complained of arm pain, that she received a report about the fall, and that swelling was present when she took over his care. She was the one on duty when the fracture diagnosis came back.
The inspection was filed as a complaint investigation. CMS rated the level of harm as minimal.
R1's arm was broken. His eye was bruised. He paces the unit every day.
