The information board near the front entrance pointed to the screening desk. The screening desk had nothing. Members of the resident council, when asked, said the binder was kept at the nurses' station. The inspector went to the nurses' station.
The administrator produced the binder from inside the nurses' station at 4:13 p.m. Then came the detail that made the location matter: residents were not allowed to enter the nurses' station. The administrator acknowledged the binder should have been sitting on the counter, within reach, not stored behind a barrier residents couldn't cross.
Ten minutes later, at 4:23 p.m., the administrator identified a second problem. The binder didn't contain the results of the 2024 annual recertification survey at all.
Inspectors rated the violation as minimal harm or potential for actual harm, affecting many residents.
