The resident, identified in inspection records as Resident 6, told inspectors on August 20 that they had not been bathed since at least August 1. Their shower was scheduled three times a week, on Tuesdays, Thursdays, and Saturdays. Across nearly three weeks, not one of those showers was given.
"They never refused a shower," the resident told inspectors.
Their representative was more direct. Staff, the representative said, would write "refuse" on the shower sheet. That was false.
The nursing assistant assigned to Resident 6 confirmed what the records showed. CNA 1 told inspectors on August 21 that the resident's schedule called for showers on Tuesday, Thursday, and Saturday, and that they did not see any showers documented for the period running from August 1 through August 19.
That is nineteen days. Eleven scheduled showers. None given. All of them marked, according to the resident's representative, as refusals the resident never made.
The way refusals were supposed to work, according to LPN 1, was that a nursing assistant would tell the nurse when a resident declined a shower, the nurse would encourage the resident to reconsider, and then the nursing assistant would write the refusal on the shower sheet. No resident signature. No second verification. Just a staff notation, unchecked.
LPN 1 acknowledged the facility had been planning to change that. The new process would require residents to sign off on their own refusals, creating a record that could not be written without the resident's knowledge. But as of August 21, the day before inspectors completed their review, that process had not been put in place.
The Director of Nursing told inspectors she expected staff to follow resident shower schedules and provide showers.
That is what she expected. What happened was something else.
The inspection tagged the deficiency under F0677, which covers basic personal hygiene and grooming. Inspectors rated the level of harm as minimal harm or potential for actual harm, and noted that some residents were affected.
What the report does not resolve is how many other refusals on that shower sheet, or on sheets for other residents, were written the same way. The facility's own nursing staff described a documentation system with no mechanism for a resident to dispute what was written about them. A resident could be recorded as refusing care they never refused, and there was no signature, no countersignature, no process that required the resident to be part of the record at all.
Resident 6 spent the better part of three weeks without a shower in an Oklahoma summer. They were scheduled to receive one every other day. They say they never turned one down.
The fix the facility had in mind, requiring residents to sign their own refusals, was a reasonable one. It was also still sitting on a list of things to do when inspectors walked in.
