The breakdown happened in the early morning hours of an unspecified date, when a chain of communication failures left an agency nurse in charge of a situation nobody had briefed him on. During the overnight handoff, the departing nurse told him there were no residents with morning appointments. That was wrong.
When the front desk called V8 around 7:05 a.m. to say the resident's ride had arrived, he scrambled. He asked a nurse whose name he couldn't recall what to do. He was told to print a face sheet. He found a CNA whose name he also couldn't recall and asked them to get the resident ready. The CNA put her in a wheelchair and brought her downstairs. V8 said he had no information about whether she needed an escort and didn't know where to find the communication book.
The lead receptionist, V10, knew the resident was supposed to have an escort. The escort wasn't available. So V10 turned to V11, a CNA supervisor, and asked him to go instead.
V11 had never done this before. When they arrived at the methadone clinic, he stayed outside. He did not go in. He waited until he saw the resident come out with the transportation driver, then got in the van and rode back. He said he didn't know what his role was supposed to be, because he had never been trained as a resident escort.
The facility's own list of residents on methadone, dated September 18, 2025, specified that this resident must have an escort. Its written transportation policy stated the facility would provide one if no family member or representative was available.
Nobody did.
