Inspectors touring the facility's main kitchen that morning found black buildup packed into the slots of removable plastic shelves inside the walk-in cooler. Two sandwiches sat on a cooler shelf with no label and no date. A glass two-door cooler had dried spills and debris along its interior base. A food preparation table's lower shelf held pans filmed with dried food, grease, and dust, some of it spreading onto the sides of the pans themselves. A clear plastic container on the same shelf held a tan substance with a scoop pushed down into it. It was not labeled. The food service director identified it as potato flakes.
Two days later, inspectors moved to the resident food pantry between Units 3 and 4. Dried food, debris, and pieces of hair were stuck to the interior of the refrigerator and freezer. A large area of dried orange-brown substance had collected beneath the refrigerator's lower drawer. Cabinet drawers holding coffee filters and unwrapped plastic utensils were dusty. A plastic tub under the sink's drainpipes contained a dried yellowish substance. A lower cabinet where straws were stored was coated in dust. Another cabinet held a loose plastic cup and lid sitting in dirt alongside a dead insect. The top shelf of a cabinet where sanitizing wipes were stored had two large spots of black substance on it.
The findings were reviewed with the nursing home administrator and director of nursing that afternoon.
It was not the first time. The same food storage violation had been cited at the facility in October 2024, less than a year before inspectors found the expired flour still in the bin.
