Inspectors touring the main kitchen that morning with the food service director found black buildup down in the slots of the plastic shelves lining the walk-in cooler. Two sandwiches sat on a cooler shelf with no label and no date. The interior base of a glass two-door cooler held dried spills and debris. The lower shelf of a food preparation table, where cooking pans were stored, was coated in dried food, grease, and dust that had spread onto the sides of the pans themselves. A clear plastic container on the same shelf held a tan substance with a scoop buried in it. It was not labeled. The food service director said it was potato flakes.
Two days later, inspectors checked the resident food pantry between Unit 3 and Unit 4. Hair was stuck to the interior of the refrigerator and freezer. A large dried orange-brown substance had pooled under the refrigerator's lower drawer. Cabinet drawers where coffee filters and unwrapped plastic utensils were kept held dust and debris. Under the sink, a plastic tub sat beneath the drainpipes containing a dried yellowish substance. A lower cabinet held two boxes of straws alongside dust and debris. In another cabinet, a loose plastic cup and lid shared shelf space with dirt and a dead insect. On the top shelf of a cabinet storing sanitizing wipes, two large spots of black substance flanked the container.
The findings were reviewed with the nursing home administrator and director of nursing that afternoon.
It was the second time in less than a year the facility had been cited for the same violation. Inspectors had documented the identical food storage deficiency on October 18, 2024.
