Federal inspectors flagged the violation on September 17, 2025, after reviewing medication administration records for a resident whose 6pm medications included Metoprolol Tartrate 40 mg and Anucort-HC Rectal Suppository 25 mg. Both were documented as given at 9:57pm.
The nurse, identified in the report as V3, a Licensed Practice Nurse, told inspectors she knew the rules. She said she was supposed to sign the electronic Medication Administration Record right after giving medications, not hours later. She acknowledged it was possible she had opened the eMAR at 6pm, given the medications to the resident, and then signed off during the 9pm medication pass instead.
"She is not supposed to wait in documenting medication administration," the report noted.
A supervisor, identified as V2, confirmed the standard during a separate interview the same morning. Nurses are expected to document immediately after administering medications, V2 said. Waiting three hours or more defeats the purpose, which is to confirm that medications were given when they were supposed to be given.
The facility's own medication administration policy required nurses to return to the medication cart after giving drugs and document the administration in the record at that time.
Inspectors classified the violation as causing minimal harm or potential for actual harm, with few residents affected. The complaint-based inspection covered a narrow set of circumstances, and the report does not describe any injury to the resident whose record showed the delay.
What it does show is a nurse who understood the requirement, couldn't account for why she hadn't followed it, and offered only the possibility that she had given the medications on time but logged them hours late. For a resident taking a cardiac medication on a twice-daily schedule, the difference between a documented 6pm dose and a documented 9:57pm dose is not a paperwork problem. It is the only record anyone has.
