Complete Care at Harston Hall: Dignity Violations - PA
The resident, identified in inspection records as Resident R1, was admitted to the facility on August 13, 2025, with sepsis, paraplegia, and a pressure ulcer. He is cognitively intact, scoring a perfect 15 on a standardized mental status assessment. He understood exactly what was happening to him.
Inspectors watching a dressing change on September 17, 2025, at 12:30 p.m. observed a licensed practical nurse leave the man with his buttocks exposed for approximately two minutes while preparing supplies. During that time, multiple staff members entered and exited the room. None introduced themselves. None ensured his privacy.
Fifteen minutes later, inspectors interviewed him.
"People come and go while I am getting care, I don't know who they are, they do not introduce themselves," he told them. "Sometimes it's someone dropping off a tray, other times it is housekeeping. It's uncomfortable when I am exposed and have no privacy and it doesn't seem like anyone cares."
He said it happens often.
The facility's own written policy, in place since September 1, 2024, requires staff to explain procedures to residents and screen for privacy before beginning wound care. The nurse performing the dressing change did neither.
Inspectors cited the facility for failing to treat the resident with dignity and respect, noting the violation affected few residents and caused minimal harm. That classification captures the regulatory finding. It does not capture what it means to be a paralyzed man, unable to cover yourself, watching strangers walk past while you wait.

Data source: This article is based on inspection data downloaded directly from the Centers for Medicare & Medicaid Services (CMS) via Medicare.gov. CMS releases inspection reports in bulk; we publish the findings as documented by state surveyors in the official Form CMS-2567 Statement of Deficiencies.
Plan of correction: The CMS report we receive does not include the facility's plan of correction. Facilities submit plans of correction separately to state survey agencies and those responses may not be reflected in CMS data at the time of publication. The absence of a plan of correction in our data does not mean one was not filed. Readers who want information about corrective steps taken are encouraged to contact the facility directly or their state survey agency.
Corrections may have occurred: Inspection reports reflect conditions observed on the date of the survey. Facilities may have implemented corrections, staffing changes, additional training, or other remediation since the report was issued. We report what CMS provides and encourage readers to seek current information from the facility.
