Her diagnosis list included lung cancer, chronic gastric cancer, emphysema, tobacco use, a personal history of pulmonary embolism, anxiety disorder, and two parasitic conditions, ocular myiasis and aural myiasis, affecting her eye and ear. She also had an injured right shoulder.
Her care assessment, completed on August 29, had a section for discharge planning. It was blank.
A separate federal assessment form showed her overall goal was clear: she wanted to be discharged to the community. That goal had been recorded. It just hadn't been acted on.
The facility's own transfer and discharge policy, updated in November 2024, required that every resident's care plan include their goals for admission, their desired outcomes aligned with discharge, and documented discussions with the resident or their representative about leaving. None of that existed for this woman.
When the administrator and director of nursing spoke with the inspector that afternoon, they acknowledged that residents should have a discharge plan in place.
The woman with cancer was still at the facility, still uncertain about her future, still waiting for someone to sit down with her and map out what came next.
