The Director of Nursing went to assess the resident herself and found what she described as a clean slit, consistent with a surgical wound, hidden inside the resident's abdominal folds. Press on it, and it opened. The resident was sent to the emergency room by ambulance.
Hospital records from August 25, the day of the resident's surgery, documented a transverse incision in the right lower quadrant. The facility's own admission skin assessment, completed that same day, documented no such wound.
The Director of Nursing said she could not fully read the skin assessment because it was handwritten and some words were illegible. She said she was not certain who completed it, though she noted the admitting nurse typically performs that assessment. The nurse assigned to care for the resident on the day of admission was reached by voicemail on September 13. She had not called back.
The wound was not the only thing missed. The resident's family member had sent an email to the facility administrator on August 27, documenting that the resident had been left soiled for an hour and a half. The Director of Nursing said she was never made aware of that email, and had not known the resident waited that long to be changed. "It is not acceptable for any resident to have to wait an hour and a half to have care rendered," she told inspectors.
The resident, who had just undergone abdominal surgery and was managing an ileostomy, kept asking staff what was happening while the wound leaked.
