Inspectors were watching when the nurse, identified in the report as V13, prepared noon medications. She poured Sucralfate, prescribed for a resident with gastrointestinal hemorrhage, into a cup held in the air. Held that way, it looked like 10 milliliters, exactly the ordered dose. When she set the cup on a flat surface, it read 15. Only after an inspector asked her to check did she remove the extra 5 milliliters and give the correct amount.
Minutes later, V13 went to give another resident their Hydroxyzine and found none in the cart. She reordered it from the pharmacy and told the resident it would arrive that evening. She marked the medication pass complete. She did not call the resident's physician to report the missed dose.
The third error was worse in one respect: it had already happened twice before inspectors caught it. V13 pulled a bottle of Simethicone from the cart's top drawer and prepared two tablets for a resident who needed two 80-milligram chewable tablets. The bottle was labeled 125 milligrams. She said she hadn't noticed the difference. When inspectors flagged it, she searched the cart, the medication room, and other floors. The facility had no 80-milligram Simethicone anywhere in the building. She acknowledged she had given the same wrong-dose tablets to the same resident that morning.
The Director of Nursing, V2, told inspectors that nurses are responsible for the right patient, right medication, right dosage, right route, and right time, and that physicians must be notified whenever a medication is held or not administered.
V13 had not done that. Neither error had been caught before inspectors arrived.
A pharmacist consulted during the inspection said that giving a resident more Risperidone than ordered would cause increased sedation and systemic slowing. The Sucralfate error had already been administered and swallowed before anyone checked the cup.
