Helia Healthcare of Energy: Wrong Medication Sent Resident to ER - IL
The incident happened on August 30, 2025. Around 8:05 that morning, the resident, identified in the report as R1, received eight medications that belonged to someone else, including two antidiabetic drugs, Januvia 100mg and Metformin 750mg, along with a diuretic, aspirin, a stool softener, iron, gabapentin, fish oil, and vitamin D3. None of them were hers.
By the time EMS arrived, her blood glucose had fallen to the 40s. The local EMT who responded, identified in the report as V17, told inspectors he had been called to the facility twice for the same resident. He said the wrong medications administered to her were documented on his run report. The EMS record listed the chief complaint as "Accidental Medication Administration Multiple."
Staff gave her oral glucose before the ambulance arrived, which brought her level up to 77. It wasn't enough. She was transported to the hospital, admitted, and given intravenous dextrose. She stayed overnight. Her hospital discharge summary described her as a woman with diabetes who had also been discharged from that same hospital just the week before, recovering from a wound infection and still on antibiotics.
She was back at the nursing facility by August 31.
The Assistant Director of Nursing told inspectors an investigation was started and that the nurse who made the error had been educated on medication administration. When inspectors asked for the facility's medication error policy on September 5, the Assistant Director said she would check but wasn't sure one existed. No policy was provided before the inspection closed on September 12.
The facility's medication administration policy, which was undated, called for a triple check of the right resident, drug, dose, route, and time before giving any medication. The nurse who gave R1 another resident's full morning regimen completed none of those checks correctly.
