The facility's own staffing targets call for three to four nurses and four to five CNAs on the first floor during day and evening shifts, with similar ratios across the second floor, third floor, and memory care unit. In August 2025 alone, the facility ran 22 shifts with fewer nurses or CNAs than those minimums.
A second CNA told inspectors that when no one picks up an open shift, the CNAs already working are simply expected to absorb the shortage. Residents miss showers. Call lights go unanswered for long stretches. "Some residents aren't happy when they're not showered when it's scheduled," the first CNA said.
The scheduler confirmed that managers and restorative aides are supposed to help fill gaps but are not taking full team assignments. She said she herself could step in on the floor as a CNA if needed.
When inspectors asked the administrator about the facility's staffing policy, the administrator said there wasn't one.
No written standard. No floor-level requirement the facility was formally obligated to meet. The staffing ratios the scheduler described existed in practice, tracked on a schedule where initials meant you worked, check marks meant you worked but forgot to initial, and lined-out names meant you'd been moved to another unit entirely. Twenty-two times in a single month, the numbers behind those marks fell short.
For the residents waiting on a call light, or told their shower would have to wait until tomorrow, the shortage was not a scheduling notation. It was the shift they lived through.
