The resident, identified in inspection records as R88, made the request in September 2024. The facility's own discharge planning protocol says that when a resident asks to transfer, staff should initiate a referral to the facility of choice. That didn't happen. Not in September. Not in October, November, December, or January. The Social Services Director confirmed to inspectors that no referrals to appropriate entities were made between September 2024 and February 24, 2025.
The facility held an Interdisciplinary Care Meeting for R88 on June 24, 2025, noting that referrals were "continuing to be completed for other facility options." But the clinical record still lacked evidence that R88's care plan had been updated to include the resident's discharge goals, preferences, or any response to information received from referrals.
When a surveyor interviewed the Director of Nursing on August 27, she said there was no specific policy for discharge planning when a resident requests a transfer to another facility. She added that the expectation to make referrals had been made clear to the Director of Social Services, and that referrals had been made since that conversation in February. The Administrator, in the same interview, pointed to language buried inside the facility's care plan policy as the governing discharge procedure.
The facility's written policies were not the problem. A comprehensive care plan policy revised in December 2024 explicitly requires the care plan to include a resident's stated preference for discharge and any referrals made to support that desire. An undated discharge planning protocol directs staff to initiate a referral when a resident wants to transfer. Both documents said the right things.
R88 spent those five months waiting, their request unacted on, their care plan unchanged, their options not explored.
