The Director of Nursing, during a tour on August 27, confirmed that every room in the hallway had tan stains surrounding the air vents. She confirmed the dark gray speckled areas in two residents' rooms and the black speckled patch on a third resident's ceiling. She said the damage appeared to come from water or moisture and that the maintenance supervisor had been trying to reach corporate for a solution.
The maintenance supervisor explained what was happening: fans in the ceiling were drawing warm outside air in, where it met cool interior air and produced moisture that dripped through the vents onto floors and ceilings below. He had open work orders for the dripping rooms. He told inspectors he had been in contact with the corporate President of Maintenance for about a month regarding the moisture problem.
Nothing had been finalized to correct it.
The facility's room roster showed residents in thirteen rooms along that hallway. One resident confirmed to inspectors that a large black speckled area had formed on the ceiling of a neighboring room, and that gray scattered patches had appeared on the wallpaper and ceiling directly above that resident's own bed, where a vent blew overhead.
The administrator in training provided the room roster. She did not appear in the inspection report as having offered any timeline for repairs.
