On August 13, 2025, a certified nursing aide at The Haven of Farmer City transferred a resident known in inspection records as R1 without using the mechanical lift that R1's care plan had required since admission. When a second aide, identified as V6, entered the room and warned against lifting by hand, the first aide, V5, had already started. V6 helped finish the transfer anyway. The moment R1 reached the wheelchair, the resident began screaming.
A licensed practical nurse arrived to find R1 sitting upright in the wheelchair, screaming "it hurts, it hurts, don't touch it." When the nurse asked what hurt, R1 said: my right shoulder. R1 would not allow an assessment. The facility sent R1 to the emergency department.
The LPN later told inspectors that V5 had seen the sling positioned under R1 and said, according to the LPN's account, "they weren't going to use that, we don't have time."
The Director of Physical Therapy confirmed to inspectors on August 22 that R1 required a two-staff mechanical lift for every transfer, and that a stand-and-pivot move was never appropriate. "There is a reason they are a mechanical lift transfer," the director said.
Staff had access to a facility cheat sheet listing each resident's transfer status, maintained by the Assistant Director of Nursing and documented in the CNA communication binder. V6 told inspectors V6 had consulted that sheet. V5 apparently had not, or had and ignored it.
The facility held an in-service on transfer policy the same day R1 went to the hospital. R1 was discharged and returned to the facility. The inspection report does not say what the emergency department found wrong with R1's shoulder.
