The facility's nurse practitioner, identified as V6, said in a statement on August 20 that she had never been informed the wound was large and actively draining. She had been told it was a skin tear. A skin tear, she explained, is superficial and non-significant. A weeping, draining wound of this size requires evaluation and treatment to prevent infection. Had she known, she said, she would have sent him out immediately.
The receiving nurse told inspectors the same thing: proper notification would have meant earlier intervention.
The inspection, triggered by a complaint, was conducted on August 22, 2025. CMS assessed the level of harm as minimal, a designation that applies to the regulatory finding, not to R1, who left in an ambulance on his first day.
