Both residents were fully dependent on staff for toileting hygiene, meaning they could do nothing themselves. One, identified in inspection records as R5, carried diagnoses of Alzheimer's disease, dementia with behavioral disturbance, hallucinations, Parkinson's disease, and seizure disorder. Assessment records noted both short-term and long-term memory problems. R5's care plan was direct: barrier cream to areas exposed to moisture, pericare after every incontinent episode.
The second resident, R6, had a BIMS cognitive assessment score of 6, placing them in the severely impaired range. R6's diagnoses included Alzheimer's disease, epilepsy, and dementia. The care plan called for barrier cream with incontinence care and assistance with personal hygiene as needed.
