The administrator, identified in inspection records as V1, told the inspector on May 28 that she sometimes sourced linen from a warehouse attached to the facility's corporate office. She could not provide any proof. She hadn't ordered the linen directly, she said.
That same afternoon, the assistant director of nursing walked the inspector to a second linen storage room in the basement. She used a key to get in. Inside were three sealed packs of new washcloths, 50 to a pack, 150 total. The assistant director of nursing acknowledged she already knew about the condition of the towels residents were using. The facility, she said, was constantly ordering towels.
The new washcloths stayed in the locked basement room while residents dried off with rags.
