Lunch for those residents had started 48 minutes late and taken nearly an hour to complete.
The facility had posted its own schedule on the wall. Noon for the small dining room. 12:20 for the large dining room. Carts staggered out to the halls at 12:35, 12:40, and 12:50. On the day inspectors watched, not one of those times was met. The 400 hall cart arrived at 1:10, 35 minutes late. The 300 hall cart at 1:17, 37 minutes late. The 100 and 200 hall cart at 1:32, 42 minutes late.
Inspectors came back the next day. At 12:07 PM, residents were sitting in the small dining room again, waiting. The cart arrived at 12:30, 30 minutes late.
The residents who spoke to inspectors said this was not unusual. One resident, identified in the report as R7, had been living at the facility since November 2025. Cognitively intact, with a mental status score of 14 out of 15, R7 told inspectors on May 20 that breakfast usually came on time but the noon and evening meals were sometimes 30 minutes to an hour and a half late. R7 said it happened consistently.
R5, also cognitively intact, had been admitted in December 2025 and told inspectors the same thing on the same day: meals delivered to their room were always late.
R2 had been a resident since July 2017 and ate in the large dining room. She told inspectors on May 21 that the noon and evening meals were often served late there too.
A licensed practical nurse, identified as V10, confirmed it. She told inspectors at 1:47 PM on May 21 that meals were served late often, that a couple of weeks before the investigation a meal had come an hour and a half behind schedule, and that she did not know why it kept happening. The schedule was posted. The delays kept occurring anyway.
A certified nursing assistant, V8, was even more direct. At 12:35 PM on May 20, while the noon meal was already more than 30 minutes overdue, V8 told inspectors this was a common occurrence. She said when meals ran late, it put staff behind on everything else they were supposed to be doing.
Sixty-two residents lived at the facility at the time of the inspection.
The violation was cited at the minimal harm level, meaning inspectors determined no resident had suffered documented injury as a result of the delays. But the citation covers something more than a logistical inconvenience. Residents with moderate to no cognitive impairment described sitting and waiting, day after day, for meals that the facility's own posted schedule said should already be in front of them. A nurse on staff couldn't explain why it kept happening. A nursing assistant said it made everything harder.
