R15 told inspectors what it felt like. She said it makes her want to cry. She said it makes her anxious, upset, and that she feels pretty bad sitting in her own waste while waiting on staff. She said certain staff do not want to help her or answer her call light at all.
The problem extended beyond that one incident. R15 told inspectors that some staff refuse to let her take a shower and force her to take a bed bath instead, because she requires a mechanical lift to get into her wheelchair and then into the shower chair. Staff, she said, do not want to do the lifting. "She wants to cry when staff do not want to assist her in the shower chair," inspectors wrote, "and does not understand why staff will do things for other residents and not for her."
The facility's own records show the call light problem was not new and not unknown to management.
Resident council minutes from April 1, 2026, documented that CNAs were not answering call lights in a timely manner. A grievance summary from March 4, 2026, recorded residents telling the council their call lights were staying on for long periods without response. The same complaint appeared again in the April 1 grievance summary.
The pattern was documented, in writing, by the residents themselves, more than seven weeks before inspectors arrived.
The facility's Medical Director, identified as V24, told inspectors on May 22 that "it is possible for a resident to feel a lack of dignity and sadness when being left in urine and feces for extended periods of time." The Director of Nursing, V2, said she expects residents to be cleaned up as soon as staff are made aware, and that she hopes call lights are answered immediately, with a maximum wait of two to three minutes, because there is enough staff in the facility to handle it.
V2 said she expects staff to treat residents with dignity and to treat them the way they would want themselves and their own family members to be treated.
What R15 described was the opposite of that. A woman who cannot transfer herself, who depends entirely on staff to keep her clean and dry and to move her from her bed to her wheelchair to the shower, was left sitting in waste for two hours across a single incident, threatened when she asked for help, and denied showers because staff found the mechanical lift inconvenient.
The citation carries a finding of actual harm. Not potential harm. Not a risk of harm. Actual harm, to a specific resident, documented by name.
Lakeside Health & Rehab is contesting that finding.
