Metropolis Rehab: Wrong Chemicals, No Safety Policy - IL
When inspectors visited Metropolis Rehab & Health Care Center, they found that none of the cleaning chemicals the facility actually had were compatible with its own dispensers. Staff had improvised. Sanitizer buckets were mixed with dish soap. For surface cleaning, workers used spray bottles of multipurpose cleaner that a housekeeper had sourced from the housekeeping supply.
The facility's own maintenance vendor confirmed it. The vendor, identified in inspection records as V3, told inspectors the facility had no bottles of any chemical that would work with the wall-mounted system and described the workarounds staff had adopted to keep cleaning going.
Sixty-four residents were living in the facility at the time, according to the midnight census taken on April 28.
When inspectors spoke with the administrator on April 29, the explanation offered was not a plan to fix the dispensers or restock compatible chemicals. The administrator said the facility had no environmental policy for maintaining its equipment. There was nothing written down requiring that the dispensers work, that compatible chemicals be on hand, or that staff use anything other than whatever was available.
Dish soap is not a disinfectant. Multipurpose spray cleaners sourced from a housekeeping closet are not substitutes for sanitizing systems designed to control infection in a building where dozens of medically vulnerable people eat, sleep, and receive care. The distinction matters most in the spaces where it is hardest to see — shared surfaces, common areas, the equipment staff touch between residents.
The inspection classified the harm level as minimal or potential, rather than actual. For 64 residents living inside a facility that could not answer the basic question of what its staff were using to clean, that distinction offers limited comfort.
