The certificate Cook A held showed a completion date of April 15, 2023, and an expiration date of April 14, 2026. By the time inspectors arrived on April 29, the certificate had been expired for two weeks. Cook A was on the schedule to work that afternoon.
The Dietary Manager told inspectors he was new and had not noticed the certificate had lapsed. He said Cook A was instructed to complete the food handler training before returning to duty.

The next morning, April 30, the Administrator produced a certificate showing Cook A had completed a food handler program through The Always Food Safe Company, with a completion date of April 22, 2026, valid through April 22, 2029.
Inspectors checked the online verification database. It told a different story. According to the database, Cook A completed the same program on April 29, 2026, not April 22, with certification valid through April 29, 2029. That is the same day inspectors first arrived and raised the issue.
The Administrator said she was unsure why the dates were different.

Inspectors cited the facility for failing to ensure dietary staff held current food handler credentials, noting the lapse placed residents at risk for foodborne illness and for not having their nutritional needs met. The violation was rated at the lowest level of harm, minimal harm or potential for actual harm, and affected few residents.
The facility's own policy, revised January 1, 2026, required dietary employees to obtain food handler permits within 30 days of hire. Cook A had been employed for more than three months before the expired certificate was flagged, not by the facility, but by inspectors conducting a complaint investigation.
What the facility did not explain was why a cook who started in January was still carrying a certificate that predated his hire by nearly three years, and why nobody in dietary management caught it in the weeks after it expired on April 14.
The Dietary Manager's explanation, that he was new and had not noticed, leaves open the question of who was responsible for verifying credentials before him, and whether anyone had checked Cook A's paperwork at the time of hire in January.
The date discrepancy on the replacement certificate is the detail that lingers. A document the Administrator handed to inspectors said April 22. The database said April 29. Those are not the same date, and April 29 is the day inspectors walked in.
The Administrator said she was unsure. The inspection report ends there.
