Sixteen days later, on April 27, an inspector knocked on his door and found it shut. R2 was lying in bed. The next morning, April 28, the door was shut again. This time R2 was sitting on the side of the bed, feet resting on the floor, his walker within reach.
When the inspector asked a CNA about R2's fall precautions that afternoon, the aide said they couldn't remember any. They recalled, generally, that R2 should be escorted when walking. They knew a binder at the nurse's station existed. That was it.
The Director of Nurses, asked about the closed door on April 28, said the only legitimate reasons to close it would be if care was being provided, or if R2 or his roommate had asked for privacy. She acknowledged that if R2 had requested the door closed, staff should first re-educate him about why it needed to stay open. She said she did not know of any reason it should have been shut during the inspector's two observations.
She also said all nursing staff should know residents' fall precautions, or at minimum know where to find them.
The first resident in the report, R1, had a different but related problem. Her care plan required her to wear padded underwear designed to protect her hips if she fell. She has frequent urinary incontinence. When the inspector asked a staff member whether R1 was wearing the padded underwear during breakfast that morning, while seated in her wheelchair, the answer was no. One pair was in the wash. The other wasn't clean.
The Director of Nurses confirmed that if R1 is out of bed, she should have the padded underwear on.
The facility's own fall prevention policy, dated November 2017, states that safety interventions will be put in place for each resident identified at risk, and that all assigned nursing personnel are responsible for ensuring those precautions are consistently maintained.
R2 sat on the side of his bed, feet on the floor, walker within reach, door shut, on the morning of April 28. No one had checked on him. No one had opened the door.
