Torpenz and Torsemide appeared side by side in the results. The nurse selected Torpenz. The order went to the pharmacy through the computer. The pharmacy filled it.
RN A told inspectors on April 29, 2026, that she did not read the order back before saving it. She said the resident frequently complained of dry mouth.
The facility notified the resident's primary care physician of the error on April 15, 2026. The resident was sent to the hospital for evaluation and returned. A week later, the resident was hospitalized again, though the physician said that second admission was for an unrelated acute medical condition. The physician told inspectors there was no negative outcome from the Torpenz, and added that the resident "complains all the time of anything and everything" and had been "waxing and waning" in behaviors.
The physician also said the pharmacy should have caught the error before filling it. The administrator agreed, telling inspectors the pharmacy failed to flag that Torpenz had been ordered for edema, which was not an approved use for that drug.
A Root Cause Analysis committee, which included RN A, concluded the cause was user error. Following the investigation, the facility required all nurses to re-read orders before saving them and added a new step: every order must now be reviewed and approved by the Director of Nursing before it is sent to the pharmacy.
RN B, who had worked at the facility for only three days at the time of the inspection, told inspectors she had already received that training.
The resident, who had been complaining of dry mouth before the wrong medication was ever ordered, was back at the facility when inspectors arrived.
