Briar Place Nursing: Roommate Notice Failures - IL
An April 2026 complaint inspection found the facility placed Resident 1 into a room with Residents 5, 6, and 7 without providing written notification to any of the existing residents or their representatives beforehand. One resident said she could not recall being introduced to her new roommate before the move happened. A second said she did not recall whether staff had introduced the incoming resident to the room at all.
The Assistant Administrator told inspectors she personally brought Resident 1 into the room and that all three existing residents were present. She said they were told they would have a new roommate. But she acknowledged no notifications were documented, no written notice was provided before the move, and no family members or powers of attorney were contacted in writing.
"If it is not documented then it never happened," the Assistant Administrator said.
The Psychiatric Rehabilitation Services Director and her assistant both confirmed they had not notified the family representative for Resident 1 of the room change, and had not notified the families of the three existing residents either. The assistant director said she was not at the facility when the move occurred.
The facility's own room change policy required Social Services or the Admissions Director to notify residents and families before a transfer and to document that notification in progress notes. If a family member could not be reached, a voicemail was to be left. None of that happened here. The facility's Statement of Resident Rights, meanwhile, guaranteed each resident the right to receive written notice, including the reason for the change, before their room or roommate was changed.
Three residents and at least one family representative went without that notice. The facility, by its own administrator's account, had no documentation to show otherwise.
