The contamination at Majestic Care of Kent extended to exposed electrical wires protruding through a six-by-six-inch hole in the wall, which were also covered in the black substance along with dirt and debris. Kitchen Aide #506 told inspectors she wasn't aware of the mold-like growth on the walls.

Maintenance Director #536 confirmed the black substance and explained the wall opening was created during installation of a new automatic dishwasher but was never properly sealed. The exposed area allowed mold and debris to accumulate inside the drywall and on the wiring.
In the walk-in freezer, inspectors discovered large bags of beef patties, chicken breasts, breaded chicken tenderloins, and peppers and onions that were all opened but carried no dates. Kitchen Aide #506 and another facility staff member acknowledged that opened frozen foods should be sealed and dated to prevent freezer burn.
The facility's own policies required clear labeling and dating of all food items to maintain safety and prevent spoilage. Kitchen sanitation policies mandated that equipment remain "clean, organized, and free of spills, mold, or build up."
Nobody had.
The violations emerged from two separate complaints filed against the facility. Inspectors classified the harm level as minimal but noted the food safety failures had the potential to affect every resident who received meals prepared in the contaminated kitchen.
The facility census stood at 51 residents during the January 28 inspection.
