The nursing assistant, identified in the inspection report as V3, told inspectors what she saw: R1 sat down immediately and buttoned his pants when he noticed her. R2, the woman in the wheelchair, sat with her mouth open. Her dementia left her, in the words of the facility's own doctor, unable to defend herself.
The facility's administrator told inspectors that had staff known about R1's hypersexual behaviors before admission, they would have requested more information, and probably would not have accepted him at all. If they had admitted him, the administrator said, it would have been to a private room with a one-on-one staff member assigned to monitor his behavior.
None of that happened. The Vice President of Business Development told inspectors the admission screening was never done at bedside. It was worked through a case manager. She said she was not involved in the front end of the process and was only looped in after the fact. "There were gaps," she told inspectors.
The company's response, she said, was education with the team. A dedicated person for admission screening in Peoria would start January 5, 2026, more than three weeks after R2 was assaulted.
The facility's doctor confirmed what the inspection report made plain: R2's dementia made her vulnerable. She could not communicate. On December 23, ten days after the assault, an inspector observed her sitting in her wheelchair, mouth open, unable to speak.
