When inspectors reviewed R3's October 2025 TAR, they found gaps. Some weekend entries were blank. No initials. No confirmation the treatments had been given.
A nurse, identified in the report as V8, walked inspectors through what that means. V8 explained that nurses are the ones performing treatments on weekends. Then V8 said something direct: if it is not documented, then it is not done. V8 confirmed, after reviewing the TAR herself, that some of the weekend treatments were not signed off and should have been.
V8 also said why it matters. Residents need to receive their wound treatments as ordered so the wounds don't get worse or become infected.
A pressure ulcer on the ankle of a resident with hemiplegia, limited sensation on her affected side, and documented mechanical risk factors is not a wound that tolerates missed care quietly. Wounds like this can deteriorate without consistent treatment, and a resident who cannot feel her foot normally may not be able to signal that something is wrong.
