The medication was already there.
A hydrocodone-acetaminophen tablet, 5-325 milligrams, had been ordered for the resident that same day, July 25, 2025, with instructions to give one tablet by mouth every eight hours as needed for pain. The facility's own medication dispensing system had the drug in stock. The Electronic Medication Administration Record shows she never received it that night. The Health Status Note, signed by LPN #1 at 9:30 p.m., reads: "Resident is complaining of pain at this time, but no medications are on hand for her."
That last part was not true. The medication was on hand. It was in the dispensing system inside the building where she was lying in pain.
The resident, identified in inspection records only as Resident #1, was admitted to the facility on July 25. Her diagnoses on admission included cellulitis of the perineum, a pressure ulcer, pain, and chronic pain. She was not confused about what she was experiencing. A cognitive assessment completed the following week assigned her a score of 13 on the Brief Interview for Mental Status, a result that indicates she was fully cognitively intact. She knew she was hurting. She said so. The nurse wrote it down.
Federal inspectors who reviewed the case rated the violation as causing actual harm.
The director of nursing, according to the inspection report, confirmed that the nurse had failed to follow the facility's own procedure for obtaining medication for a resident in pain, and that this failure belonged to the facility.
What the inspection report does not explain is why LPN #1 documented that no medications were on hand when the dispensing system held exactly the medication that had been prescribed. Whether that was a mistake, a miscommunication, or something else, the report does not say. What it does say is that the resident went without pain relief on the night she arrived, carrying wounds and a documented history of chronic pain, while the answer to her complaint sat unused somewhere down the hall.
Cellulitis of the perineum is an infection of the soft tissue in one of the most sensitive areas of the body. A pressure ulcer adds a second source of pain. Chronic pain means she was already carrying a baseline level of suffering before she arrived. The combination is not abstract. It is the kind of physical reality that makes hours matter.
She was cognitively intact. She understood what was happening to her, and what was not.
The inspection, a complaint survey, was conducted on October 2, 2025. The deficiency was cited under F0697, which addresses the management of pain and the obligation to provide treatment and services for residents experiencing it. The level of harm was classified as actual harm, not potential harm. Not a paperwork lapse. Actual harm to a specific person.
The nurse signed her name to a note that said there was nothing available. The record shows there was. The resident spent her first night at Oxford Health & Rehab in pain that a tablet already inside the building could have addressed.
Nobody gave it to her.
