Cass County Senior Living: No Qualified Infection Preventionist - IL
Inspectors visited the 27-resident facility on September 19, 2025, and found the gap running straight through the chain of command.
The administrator, identified in the inspection report as V1, told inspectors that the Director of Nursing, V2, was serving as the facility's infection preventionist and was responsible for the infection control program. Two regional staff members, a regional nurse and a regional infection preventionist, provided oversight but came on-site only once or twice a month, the administrator said.
Then came the admission. V1 confirmed that V2 had not completed infection preventionist education.
When inspectors spoke with V2 directly at 2:00 PM that same day, the Director of Nursing didn't dispute it. "I have enrolled in infection preventionist class," V2 said. "I have not taken the classes yet."
Enrolled. Not completed. Not in progress. Not even started.
The facility's own job description for the infection preventionist role, written in October 2020, spells out what the position requires: specific training in infection prevention and control through an accredited continuing education program. The person holding the title didn't have it. The facility's 2025 assessment of its staffing needs didn't list a qualified infection preventionist as part of the plan at all.
That assessment covers what the facility needs based on its current census and resident needs. Infection prevention didn't make the list.
Infection control programs in nursing homes exist for a reason that becomes obvious whenever one fails. Residents in long-term care are among the most vulnerable to infections, often because of age, underlying illness, compromised immune systems, shared spaces, shared staff, and the close physical contact that comes with daily care. An infection that a healthy person shakes off in a week can kill a nursing home resident. The person responsible for preventing that from happening needs to know what they're doing.
At Cass County Senior Living, the person in that role was working toward the knowledge she needed. She hadn't gotten there yet.
The regional infection preventionist, V5, came through once or twice a month. That's the backstop the facility was relying on for a program that operates every day, with every resident, during every shift.
Inspectors cited the deficiency under F0882, which covers the requirement to designate a qualified infection preventionist. The level of harm was listed as minimal harm or potential for actual harm. The finding was noted as affecting all 27 residents currently living at the facility.
The facility's census report, dated two days before the inspection, confirmed those 27 residents were present when the gap was found.
There is no indication in the inspection report that the Director of Nursing's enrollment in the course was recent. There is no indication of when she enrolled, how long the course takes, or when she expected to finish.
