She described the CNA's conduct as contrary to evidence-based practice, which the facility implements, she said, to protect both residents and staff.
The infection preventionist, a registered nurse identified as RN #3, said in a telephone interview the morning after the inspection that CNA #1 should have washed his hands before care, during care, and after care. She said staff working with high-risk residents are expected to wear gowns in addition to gloves. She said she has personally conducted multiple in-service trainings on infection control and that those trainings exist for one reason: to stop infection from spreading.
The trainings happened. The knowledge was there. On the day it mattered, none of it was applied.
Perineal care, the cleaning of the genital and rectal area, is among the most direct routes for introducing bacteria into a vulnerable body. For a resident who uses a catheter, has limited mobility, or cannot signal discomfort, a single lapse in technique can set off an infection that becomes serious before anyone realizes it has started.
