Villa At Borgess Place
Kalamazoo, MI  ·  2/5 stars

That comment, made sometime around mid-July 2025, set off an investigation at Villa at Borgess Place that ended with a nurse's termination, an ongoing police inquiry, and a resident who declined to go to the hospital and has not received a public accounting of what happened to her.

The resident, identified in inspection records only as Resident 81, was described by a CNA as normally social, the kind of person who would walk the hallway to visit other residents and sing with a neighbor. She had been less herself lately, the CNA said, not feeling well, needing more help than usual.

During the shower, she told CNA J she was glad her fungal infection had cleared. Then she said she didn't feel comfortable with the wound care nurse touching her down there.

CNA J reported it. An LPN named SS also made a call. The unit manager and the Director of Nursing came in.

What the Director of Nursing, identified as DON B, learned when she sat down with Resident 81 was more specific and more serious than the initial report suggested. Resident 81 told her that a couple of weeks before the shower conversation, Wound Nurse FFF, also referred to in the report as Wound Nurse KKK, had been in her room performing an assessment. During that assessment, she said she felt his fingers inside her vagina, which he moved around.

She was offered a trip to the emergency room. She said it wasn't necessary.

DON B noted that Resident 81 did have vaginal dryness, vaginal discomfort, and pain above the pubic bone.

The wound care nurse was not in the building when the allegation came in. DON B called him and told him he was immediately suspended. He came to the facility shortly after. The nursing home administrator, NHA A, asked him to walk through what had happened during the assessment. He said he didn't want to say anything at this time. He was told to reach out if he changed his mind. He left.

NHA A called him again on Monday. He came back in. She told him the allegation. He said he didn't want to talk without representation. He left again.

He later obtained an attorney. He did not provide a statement to the facility's investigation. He did not respond to a subsequent outreach from NHA A. He was terminated.

The police were notified. As of the August 13 inspection, a detective told NHA A he was waiting for the wound care nurse's attorney to contact the police and provide a statement. That had not happened. The inspector who conducted the survey also attempted to reach the wound care nurse directly. There was no response before the inspector left the building.

The inspection was triggered by a complaint. The violation was cited at a level of minimal harm or potential for actual harm, affecting few residents. That classification reflects the regulatory framework's assessment, not necessarily the resident's experience of what she described.

Resident 81 was social. She sang with a neighbor in the hallway. She told a CNA during a shower, almost in passing, that she was glad something was better and that she hadn't liked being touched. It took that comment, and a CNA who reported it, for anyone to know what she said had happened to her weeks earlier.

She never went to the hospital. The nurse never gave his account. The detective is still waiting.
