Sherman Oaks Health & Rehab: Survey Results Hidden - CA
All seven pointed to a black metal rack outside the Activity Director's office. That's where the binder was supposed to be. When inspectors walked over and checked, the rack was empty.
The inspection, conducted March 14, 2025, cited the facility for failing to make its most recent survey results available in a place where residents, family members, and legal representatives could examine them without having to ask anyone for help.
The Activity Director walked inspectors to a second location, a table outside the social services and admissions office. The binder was there, but placed flat and tucked underneath the table's surface, with no sign marking it. The AD explained that the binder had previously sat on top of the counter but had been moved because residents were taking it to their rooms at night and writing on the papers.
The binder itself had another problem. It didn't contain the most recent recertification survey results from 2024.
The administrator, also present during the lobby walkthrough, acknowledged the results should be accessible. "The State inspection results should be available for all residents and/or their representatives," the administrator told inspectors, "so residents and resident representatives know what the facility is working on." The administrator said the survey results should be "readily accessible."
The facility's own written policy, last reviewed in September 2024, described exactly what was required: a copy of the most recent standard survey maintained in a three-ring binder, placed in an area frequented by most residents, such as the main lobby or the activity room. The binder that inspectors found was in neither location in any meaningful sense. It was out of sight, incomplete, and unknown to every resident who attended the council meeting that morning.
The violation was classified at the lowest level of harm, meaning inspectors found minimal harm or potential for actual harm rather than documented injury. But the gap between the policy on paper and what residents actually encountered was complete. Seven residents. Seven who couldn't find it. Zero binders where the rack said there should be one.
Inspection results exist so that residents and families can evaluate a facility before moving in or make informed decisions about care already underway. A hidden binder missing a year's worth of data doesn't serve that purpose for anyone.
