review of the current sampled residents revealed no concerns with facility staff failing to provide a safe, clean, homelike environment that is free from rodents in patients' beds. Throughout the course of the survey, this surveyor interviewed multiple residents, including the resident council president and vice president, observed multiple locations in the facility, and did not receive any resident or family concerns regarding this issue. This concern was discussed with the administrator and director of nursing on 10/21/25 at 6:00 PM during the exit conference. No further information was provided prior to exit on 10/21/25. This is
a past non-compliance deficiency.
