Interviews with staff were conducted and it was confirmed staff are knowledgeable on what to do in the event any pests are observed in the facility. A review of Resident Council Minutes for March, April, and May 2025 did not disclose any issues/concerns of facility staff failing to provide a safe, clean, homelike environment that is free from rodents in patients beds. A review of the current sampled residents revealed no concerns with facility staff failing to provide a safe, clean, homelike environment that is free from rodents in patients' beds.

Throughout the course of the survey, this surveyor interviewed multiple residents, including the resident council president and vice president, observed multiple locations in the facility, and did not receive any resident or family concerns regarding this issue.

This concern was discussed with the administrator and director of nursing on 10/21/25 at 6:00 PM during the exit conference. No further information was provided prior to exit on 10/21/25.
