The DON stated at the time of resident 3's incident they had a lot of agency staff in the facility.
The DON stated that residents were educated when they want to leave the facility, they were to sign the LOA book with the time they were leaving, where they were going, and a time they expected to return.
The DON stated the LOA book was located at the nurses' station.
The DON stated the residents that were alert and oriented were usually granted LOA privileges.
