jeopardy to resident health or current labs to ensure appropriate lab value levels. 3.Performance Improvement Plan (PIP) was safety initiated on 4/9/26 by the Administrator where they identified concerns, Performance Improvement Goals, Corrective Actions which included staff education, medication administration process review,
and Evaluation.
Progress will be reviewed through medication administration audits, staff competency evaluations, QAPI committee review. PIP was reviewed on 5/29/26 and reflected as completed on 4/9/26. 4.Medication Competency Assessment completed on nurses on 4/9/26.
Record review of Medication Competency Assessment on 5/29/26 showed completed and signed on 4/9/26. 5.Nursing staff were in-serviced on Seizures Symptoms and Causes of seizures, Medication Administration, Notification of changes, and Medication competency on 4/9/26.
Record review on 5/29/26 of in-services reflected in-services on Seizure Symptoms and Causes of Seizures, Medication Administration, Notification of Changes, and Medication Competencies were completed on 4/9/26.
Staff interviews on 5/29/26 between 10:40 am to 3:45 pm reflected staff were in-serviced and knowledgeable of seizure symptoms, causes of symptoms, procedure to input hospital DC orders and that they had received competency training/skills check offs.During interviews of nursing staff from all shifts on 5/29/26 between 10:40 am to 3:45 pm, LVN B, RN C, RN D, LVN E, LVN F, LVN G, and ADON revealed they were knowledgeable of the facility training provided above including seizure symptoms, causes of symptoms, procedure to input hospital DC orders and that they had received competency training/skills check offs.
