Inspection Findings
Observation and interview on 03/25/2026 at 12:31 p.m., Resident
room with the generic name of Provera written down. Resident #406 stated the case manager from
knowledge. He stated he did not sign consent for Provera. Resident #406 stated he was not going to take the Provera I don't need anything for my sex drive. I don't even have a girlfriend in this place.
During an interview on 03/252026 at 12:41 p.m., the DON stated the prescriber was responsible for obtaining the signed consent.
Requested signed consents from DON for Resident #406 Paxil and Provera.
During an interview and record review on 03/25/2026 at 12:43 p.m., the DON provided signed consent for Paxil but stated she did not have a signed consent for Provera.
She stated Resident #406 informed her he was not going to take Provera, so she was going to discontinue it.
During an interview on 03/27/2026 at 8:40 a.m., the DON stated she talked with Resident #406 yesterday about his concerns with signed consent.
She stated Resident #406 threatened to take my nursing license.
During an interview on 03/27/2026 at 8:55 a.m., Resident #406 stated he wanted to talk about this hormone thing. He stated he asked the NP 2 days ago and she told him she did not order the hormone, the psychiatric (psych) physician ordered it. He stated the psych physician talked to the nurses and NP about his behaviors. Resident #406 explained that the psych physician showed him the changes he was making to his medications and a hormone was not mentioned. He stated he had a night nurse look up the order to see who ordered the hormone and was told the NP ordered it. Resident #406 denied any adverse reactions to taking 2 doses of the hormone.
During a phone interview on 03/27/2026 at 10:57 a.m., the psych physician stated he visited Resident #406 on 03/23/2026 at the facility. He stated he did not prescribe Provera.
The physician stated he talked to the resident, educated him on the antidepressant prescribed, and briefly talked about the Provera prescribed by the NP. He stated the resident's response was whatever you think is appropriate, you are the professional and know what is best.
The physician stated he did not obtain signed consents, the facility was responsible for obtaining a signed consent form.
During an interview on 03/27/2026 at 11:21 a.m., the ADON stated the nurse that took the order was responsible for obtaining signed consent.
She explained if the nurse was unable to get the resident to sign a consent, the nurse should ask one of us to get the resident to sign the form.
The facility was unable to provide a policy on obtaining written consent to administer medication.
