During interviews on [DATE] at 4:16 PM to 4:45 p.m., on ANE return demonstration staff stated [day shift 6:00 a.m. to 6:00 p.m., [day shift CNA E, PT G, K, OT H,J, Kitchen L, LVN M, O, Housekeeping P, Admissions Y, and SW Z] and night shift (6:00 p.m. to 6:00 a.m.) [ CNA F, Q, U, R, V, X, Kitchen Aide I, Housekeeping N, and LVN Q, S, and T] staff were eligible to work weekend ] the key highlights of the training were to report immediately any acts or suspicion of ANE and to know the signs and systems, and provide safety to the resident.
During interviews on [DATE] at 4:16 PM to 4:45 PM, on de-escalation return demonstration staff stated [night shift CNA F, Q, U, and LVN Q, S, and T] [day shift CNA E, PT G, K, OT H,J, LVN M, O, and SW Z] the key highlights were to stay calm and provide safety, separate, and do not leave the victim with the perpetrator of abuse.
Record review on [DATE] of facility's grievance log for the past 90 days [February -[DATE]] revealed no grievances or incidents involving CNA A with other residents.
Record review on [DATE] of facility's incident accident log for the past 90 days [February-[DATE]] revealed no incidents involving Resident #1 with CNA A.
Record review on [DATE] of facility's policy titled Abuse, Neglect, Exploitation Statement, dated [DATE], read, .Residents of the facility shall not be subject to abuse.Abuse [was defined as] .The negligent willful infliction of injury.with resulting with physical or emotional harm or pain to an elderly or disabled person by the person's caregiver.
During exit conference on [DATE] at 5:00 PM, the Administrator and the DON were informed:This failure was identified as past noncompliance IJ as the facility had instituted adequate corrective measure to prevent recurrence of the noncompliance.
The noncompliance began on [DATE] and ended on [DATE].
The facility corrected the noncompliance before the survey began.
