Inspection Findings
Investigation revealed; it is the policy of this facility to provide protection for the health, welfare and the rights of each resident by developing and implementing written policies and procedures that prohibit and prevent abuse, neglect, exploitation and misappropriation of resident property.
Abuse means the willful infliction of injury, unreasonable confinement, intimidation, or punishment with resulting physical harm, pain or mental anguish, which can include staff to resident abuse and certain resident altercation.
Abuse also includes deprivation by an individual, including a caretaker, of goods or services that are necessary to attain or maintain physical, mental, and psychosocial well-being.
Instances of abuse of all residents, irrespective of any mental or physical condition, cause physical harm, pain or mental anguish. It includes verbal abuse, sexual abuse, physical abuse, and mental abuse including abuse facilitated or enabled through the use of technology.

During interview with LVN S on 3/26/2026 at 2:25 p.m., she stated
She stated she did not notify the Administrator of the incident, as she did not initially think of it.

She stated that staff must report any suspected or witnessed abuse immediately to the Administrator, who serves as the abuse coordinator, and stated that failing to do so could result in continued resident harm and personal disciplinary consequences.During interview with CNA Z on 3/26/2026 at 3:12 p.m., she stated that the incident was not reported to the Administrator and she was unsure why it was not reported, noting that the nurse in charge was responsible for reporting.
She stated that failure to report abuse could result in the incident continuing and could cause further harm to the residents.

Reporting of all alleged violations to the administrator, state agency, Adult Protective Services and to all other required agencies within specified timeframes: a. immediately, but not later than two hours after the allegation is made if the events that cause the allegation involve abuse or result in serious bodily injury, or b.
Not later than 24 hours if the events that caused the allegation do not involve abuse and do not result in serious bodily injury.
