witness statement, not dated, revealed she worked November 28th, 2025, on night shift and Resident 1 was asleep until 3:30 AM, and at 6:00 AM she washed Resident 1 and didn't notice any bruises at time of care.A second statement, not dated, revealed she didn't see anybody go into Resident 1's room except the nurse to administer medication. At 6:00 AM, she assisted Resident 1 into the bathroom to void, wash up and assist with dressing him; no bruises were observed at that time.During an interview with the Nursing Home Administrator (NHA) and Director of Nursing (DON) on October 22, 2025, at 11:21 AM, it was revealed that Employee 3 was assigned to care for Resident 1 on nightshift November 28, 2025. The shoehorn belonged to the Resident, and the wife felt it would be helpful as she utilized it at home. Resident 1 was provided two pictures that fit the description provided and he pointed out Employee 3 as the perpetrator. It was also revealed that Employee 3 had only worked on M2 and no other units.Review of facility investigation documentation included interviews with staff who worked November 28th, 2025, on evening and night shifts, and November 29th, 2025, on dayshift. Interviews were conducted with other residents who resided in the same unit and received care from Employee 3, and no other abuse concerns were reported.During an onsite visit, on December 22, 2025, at 9:00 AM, the NHA and DON provided the facility's plan of correction that was put into place as a result of the facility's abuse investigation. The facility's education and audits were reviewed during the survey.As of December 8, 2025, facility staff were educated on Abuse, Standards of Care, focusing on creating an environment of respect, dignity, and protection from harm and competency tests were completed.As of December 2, 2025, initial audits were completed to ensure no other residents were affected. Weekly audits were conducted for an additional 4 weeks. No additional abuse concerns were identified. Review of facility documentation revealed that on December 8, 2025, the facility had completed education and audits to ensure compliance.During the abbreviated survey, audits and staff education were reviewed. Staff interviews, resident interviews, resident
