Review of a facility provided investigative document dated March 5, 2026, included a written statement from Employee 22, Nurse Aide (NA).
Employee 22 documented he was transferring Resident 65 from a recliner chair to the bed.
Employee 22 documented that he stood in front of the resident and lifted the resident from the chair to a standing position.
Employee 22 documented that the resident was not wearing nonslip socks (footwear designed to improve traction and reduce slipping risk), but this was not identified until after the resident was already standing.
Employee 22 documented the resident slid to the floor.
Employee 22 documented he lifted the resident from the floor and placed the resident in bed and then notified the nurse to assess the resident.
Employee 22, NA, documented he was aware Resident 65 required assistance of two staff members and the use of a Hoyer lift for transfers, but he indicated that he had transferred the resident independently previously without incident.
According to the facility investigative documentation.
Employee 22 NA was immediately suspended and subsequently terminated from the facility. A review of hospital records revealed Resident 65 was evaluated for injuries sustained during the incident.
Diagnostic imaging including a CT scan (computed tomography, an advanced imaging method that creates detailed pictures of internal structures of the body) of the cervical spine (neck bones), chest, abdomen, and pelvis revealed a mildly displaced fracture of the lateral end of the left clavicle (break of the collarbone, the bone connecting the arm to the body) and an acute fracture of the left posterior first rib (break in the upper rib located near the collarbone).
Attempts to contact Employee 22 via telephone to obtain an interview on March 25, 2026, and March 26, 2026, were unsuccessful. A review of Employee 22's personnel file revealed Employee 22 signed an attestation on September 9, 2025, acknowledging completion of orientation training, which included instruction on the facility abuse and neglect policy and safe transfer and lift techniques.
During an interview on March 25, 2026, at 12:45 PM the Director of Nursing confirmed the facility investigation determined Employee 22 did not follow the resident's care plan requiring assistance of two staff members and the use of a Hoyer lift for transfers substantiating neglect.
The Director of Nursing confirmed Employee 22 was suspended pending investigation and was subsequently terminated. 28 Pa.
