Observation revealed the oxygen tank was empty.

During interview on March 28, 2026, at 11:23 AM, Employee 4, Registered Nurse (RN), confirmed the oxygen tank was empty and replaced the tank with a full oxygen tank.

Employee 4 assessed the resident's vital signs and documented an oxygen saturation level of 94 percent (measurement of oxygen level in the blood) and heart rate of 62 beats per minute.

Employee 4 indicated the resident's skin color appeared normal and there were no signs of distress.

Employee 4 confirmed the resident had physician orders for continuous oxygen and staff should monitor oxygen tank levels.

During interview on March 29, 2026, at 9:30 AM, the above findings were reviewed with the Director of Nursing (DON) and Nursing Home Administrator (NHA).

The facility failed to ensure Resident 38 received oxygen continuously in accordance with physician orders. 28 Pa.
