Honor the resident's right to a safe, clean, comfortable and homelike environment, including but not limited to receiving treatment and supports for daily living safely. NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY Based on observation, it was determined that the facility failed to provide a safe, sanitary, and comfortable environment in the main kitchen and on one of four nursing units. (St. [NAME])Findings include: On January 29, 2026, from 12:00 p.m. to 1:00 p.m. the following was observed: The mop sink located in the main kitchen's janitors' closet was clogged and with an accumulation of black sludge around the base of the sink.
