Observation of staff response to call bells revealed Resident 13 activated her call bell on March 24, 2026, at 11:28 AM, staff responded to Resident 13's call bell at 12:12 PM (44 minutes).

Further observation of staff response to Resident 13's call bell revealed on March 25, 2026, at 10:00 AM Resident 13 activated her call bell, staff responded to her call bell at 10:33 AM (33 minutes)

The facility failed to resolve the residents' grievances related to call bells.The surveyor reviewed the above findings with the Nursing Home Administrator and Director of Nursing during a meeting on March 25, 2026, at 2:40 PM. 28 Pa.

Resident interventions for this focus included providing cuing for safety and sequencing.

Review of Resident 121's Documentation Survey Report for January 2026, revealed the resident's tub/shower schedule as Wednesday and Saturday Evenings.

There was no documentation that staff assisted Resident 121 with a shower after January 10, 2026, until January 28, 2026 (17 days).

Observation of Resident 16 with Employee 1 (licensed practical nurse) on March 27, 2026, at 8:38 AM confirmed that Resident 16 continued to have supplemental oxygen in place via the room concentrator set at 4 l/m.

Employee 1 confirmed that active physician orders for Resident 16 instruct staff to use supplemental oxygen PRN at 2 l/m.

Review of Resident 16's medical record indicated that the facility failed to develop a care plan that included Resident 16's use of supplemental oxygen.

Review of Employee 5's (nurse aide) personnel record revealed that the facility hired her on July 5, 2023.

Review of training records provided by the facility for Employee 5 dated July 5, 2024, to July 5, 2025, revealed that Employee 5 completed only nine hours and 43 minutes of in-service education.
