According to Staff 18, any staff providing catheter or perineal care would notice the wound if they examined the area closely.

She described the wound as a rip, noting that discharge was visible each time the brief was changed.

She recalled the wound being on the left side of the penis and stated it resembled a split like a hot dog bun, causing the resident pain during cleaning.On 11/4/25 at 2:55 PM, Staff 2 (DNS) and Staff 3 (Resident Care Manager) stated they were not aware of Resident 1's Stage 2 pressure ulcer.On 11/4/25 at 3:10 PM, Staff 4 (Resident Care Manager) stated during his initial visit with Resident 1, the resident expressed experiencing discomfort in the penis area, describing it as a sensation of something pulling.

Staff 4 stated based on the way the resident communicated her/his concern, Staff 4 interpreted the discomfort was not current but had occurred previously.

Staff 4 stated on 9/4/25 he looked at the resident's penis and noticed a split, maybe around an inch long, which was reported to the facility provider. On 11/4/25 at 4:04 PM, Staff 1 (Administrator) and Staff 19 (Regional Director of Quality Assurance) were informed of findings for Resident 1 who was admitted with a Stage 2 pressure ulcer located on the left lateral meatus of the penis which was not identified, assessed, or care planned upon her/his admission to the facility.
