Review of the ODH Certification and Licensure System (CALS) on 09/11/25 confirmed no SRI was completed regarding the allegation of verbal abuse from staff to Resident #68.

Review of facility policy titled, Abuse, Neglect, Exploitation & Misappropriation of Resident Property, dated 2022, revealed the policy was to investigate all alleged violations involving Abuse, Neglect, Exploitation, Mistreatment of a resident, or Misappropriation of Resident Property.

Staff should immediately report all such allegation to the Administrator and to the Ohio Department of Health (ODH) in accordance with the procedures in this policy.
