Certified Nurse Aide #1 was changing the resident. The Medial Director stated the decision was made to send the resident to the hospital because there was a hematoma on the resident's head. During an

interview on 10/14/2025 at 4:03 PM, the Director of Nursing stated they remembered the resident's fall. The Director of Nursing stated because of the fall, the resident sustained a hematoma and laceration on left side of their forehead. The physician was notified, and the resident was transferred to the hospital. 10NYCR 415.12(h)(2)
