Inspection Findings
During an interview on 05/27/2026 at 1:44 PM, Family Member #2ˆstatedˆthat on the morning of 05/13/2026, they attended a care conference via telephone for Resident #1.
They stated they did discuss that Resident #1 was having fatigueˆand nausea on 05/13/2026.ˆˆFamily member #2ˆstatedˆthat on the morning of 05/14/2026, they did not receive notification that Resident #1 had had a vasovagal episode or that they vomited.
The first notification they received from the facility was on 05/14/2026 at 12:07ˆPMˆwhenˆthey were informed that Resident #1 had passed away.ˆ ˆ
During an interview on 05/27/2026 at 1:38 PM, Director of Nursing #1 stated a family member or representative would be notified for any change in a resident's baseline.
They stated family would be notified of incidents and accidents, any increase in pain from baseline, any new signs or symptoms the resident is displaying that show a change in medical condition.
During an interview on 05/27/2026 at 2:20PM, Licensed Practical Nurse #1 statedˆthat family would be notified if there was a severe change in a resident condition, death, anyˆmedication changes, when they went to see an outside provider, or for any change in the plan of care.ˆTheyˆwould consider a severe change in a resident condition to be an altered mental status, altered respiratory status, or vital signs outside of the normal range.ˆThey stated Family Member #2 was not notified of theˆvasovagal incident that occurred on 5/14/2026 when Resident #1 was in the shower.ˆˆ During an interview on 05/29/2026 at 9:53 AM, Physician #1 stated they would expect the family of a patient to be notified if there was an unexpected change in a resident's condition, especially if a resident was one who could not make their own decisions about treatment. 10 New York Codes, Rules, and Regulations 415.3 (f)(2)(ii)ˆ
